In previous trainings, we explored the history of IDD services, the structure of HCBS in Colorado, the Final Settings Rule and Member Rights. Now, we turn to one of the most critical tools for ensuring choice, autonomy, and individualized support in HCBS services: the Person-Centered Support Plan (PCSP).

This isn’t just a form—it’s a living document that reflects the voice, goals, and preferences of the person served. It is the foundation of person-centered care and the legal roadmap for how supports are delivered.
